I have a great one that i am really embarrassed to tell .........
When i first got my

in 2012, we immediately had it slipped, but on a floating rather than fixed dock. this made boarding quite difficult as the dock was at the waterline which required a step stool to board. Admiral VERY new to boating. I had engine idling with admiral at helm while i untied dock lines. Instructed admiral to slowly pull her forward and that i would "hop" on like i had done so many times in the past with other boats (lower freeboard obviously). That worked real well - As i tried to hop on while holding stanchions I ended up in the water as the boat moved away with the admiral freaking out not knowing what to do. Good thing there were no stray electrical currents in water! Eventually admiral was able to put engine in idle while i climbed aboard.
Needless to say, we changed over to a fixed dock with finger pier at about same height as deck of boat, gave the Admiral some lessons, and will never ever do that again - i have learned to remove all dock lines from onboard! I can laugh about it now - but it was a bit scary at the time. thank goodness there was no wind at the time and no other boats around (or people to see the spectacle). NEVER AGAIN !! I learned several lessons that day for sure.
Would love to hear a more embarrassing story from anyone!!

SOP for such an encounter is to pass to starBOARD then enter it in the LOGbook. Penalty for not doing so is walking the plank!

What Branch of LEO enforces this? Is there a Root cause?
